60 Minute Monthly Swedish / Deep Tissue Massage OR Rusty's Signature European Facial Membership
Rusty's Hair Salon & Day Spa is pleased to offer a 12 month massage membership for $59.00 per month and facial membership to it clients at only $75.00 per month. This includes a 60 minute Swedish, Deep Tissue Massage or Rusty's Signature European Facial. Any unused prepaid monthly fee's will move to the next month. Rusty’s Hair Salon & Day Spa
Membership Contract – Massage / Facial Services
Individual Contracts Only (Not Shareable)
Payment Terms: Twelve monthly installments for one per month 60 Minute Rusty’s Signature Swedish or Deep Tissue Massage OR one 60 Minute Rusty’s Signature European Facial service at Rusty’s Hair Salon & Day Spa. This contract is for a period of 12-consecutive months at a rate of $59.00 per month for Massage Membership and $75.00 for Facial Membership and will be automatically debited from the member’s registered credit or debit card on file each month. The payment is debited monthly on the same day of each month as the first payment (this is the sign-up date). The monthly charges will continue automatically each month for 12 months, or until the termination of the membership. Memberships will automatically move to Month-to-Month after initial 12 month contract commitment unless contract is terminated by request of the client at the end of the 12 month contract period.

o Sign-up/First Payment on 10/11/16
o First Automatic Payment charged on 11/11/16
Membership Freeze: This membership may be temporarily frozen for up to 90 days. The contract freeze will stop automatic payments scheduled during the period of the freeze from being charged and will also suspend all membership benefits during the same period. Automatic payments will continue immediately following the period of the contract freeze, as will full membership benefits. All requests for membership freeze must be submitted to Rusty’s Hair Salon & Day Spa management at least 10 days in advance of the requested freeze start date. Member’s that choose to take advantage of the membership freeze should be aware that use of this feature will extend the contract the number of days/weeks/months equal to the length of the contract freeze. This also means that you will still be contractually obligated to pay for all 12 installments in full.
EXAMPLE #2:
o Sign-up/First Payment on 10/11/16
o First Automatic Payment charged on 11/11/16
o 60 Day freeze is requested on 5/13/17 and processed on 5/13/17
o Automatic payments scheduled between 5/13/17 and 7/21/17 would not be charged.
o Membership benefits would also be suspended from 5/13/15 until 7/21/15, and would resume on 7/22/17.
o The next automatic payment would be charged on 8/11/17
Termination: To cancel this membership at any time prior to the expiration date of the contract will result in an early termination fee $100 is required. All requests for membership cancelation must be submitted to Rusty’s Hair Salon & Day Spa management at least 10 days in advance of the requested cancelation date. Once we have received and completed your request for cancellation the next scheduled monthly charge and all those that were to follow will be cancelled. Membership will be valid until, but not on, the date of the first cancelled payment; after that date, all member benefits will be canceled.
EXAMPLE #3:
o Sign-up/First Payment on 10/11/16
o First Automatic Payment charged on 11/11/16
o Membership cancelation is requested on 5/13/17 and processed on 5/13/17
o A $100 cancelation fee will be charged on 5/13/17, the date of cancelation.
o The automatic payment scheduled for 6/11/17 and all subsequent automatic payments would NOT be charged.
